The password must have between 8 and 64 characters. The password must contain at least 3 of the following:

  • a lowercase letter
  • an uppercase letter
  • a digit
  • a symbol.

At the bottom of the first page of the Queensland CTP claim portal, there is a ‘Forgot Password?’ button. Click on this and follow the instructions.

No, at this stage your password will not expire. You will only need to reset your password if you have forgotten it.

As it is an individual login, please use your individual email address. If you are using this platform in the course of your employment, please use your business individual email address.

You will need to provide either a landline or mobile phone number. If you do not have a mobile phone, you can provide a landline number and select the option to receive a call.

Please contact our team at maic@maic.qld.gov.au within 1 business day of the employee’s departure, requesting that we revoke their access to the system.

Your treating doctor completes the Online medical certificate. Take your medical certificate request ID and instructions to your appointment with your medical practitioner.

Using the system

Once your treating doctor has submitted the Online medical certificate, the person who created the Online claim form (you or your legal representative) will receive an email to confirm that. The email will be sent to the email you used to set up your Queensland CTP claim portal account. You can then log in to the Queensland CTP claim portal to view the completed medical certificate on page 14 of your draft Online claim form.

Contact your treating doctor to confirm the if the Online medical certificate was submitted. If the treating doctor confirms the submission, contact us for further assistance.

Your treating doctor can download a PDF version of the Medical certificate (PDF, 538KB) from the MAIC website. Once you have received a copy of the completed medical certificate, you’ll need to scan and upload it to page 14 of the Online claim form.

Yes, but this may cause delays in your claim.

To keep your personal information secure, your draft online request will remain in the system for 60 days after it was last modified. After you submit your request, it will remain on your dashboard for 21 days.

You can then log in to the Queensland CTP claim portal to view the completed Online medical certificate on page 14 of your draft Online claim form. Follow instructions on page 14 to lodge your claim form and medical certificate with the CTP insurer.

Yes, your doctor is required to physically examine you before completing the medical certificate.

It is likely that your treating doctor will charge for the consultation, including the time it takes to fill in the certificate. Medicare benefits may apply; speak to your treating doctor for more information.

You can instead download a PDF version of the Medical certificate (PDF, 538KB) and ask your medical practitioner to complete and give to you. The completed medical certificate will then need to be scanned and uploaded to the online claim form on page 14.
