Before you get started with your claim, review the Pre-claim checklist and ensure you have the supporting documents and information you need.
Complete and submit a claim form
Lodging a Notice of Accident Claim Form, along with any supporting documents, notifies the Compulsory Third Party (CTP) insurer of the vehicle that may have been at fault that you wish to claim and provides information they need to assess it.
Make sure you complete as much of the form as possible.
Claimants must indicate on the claim form whether they wish to settle (finalise the claim) and, if so, state the full settlement amount. If the extent of injuries is not yet known, claimants can select ‘not ready at this stage’. For more information, see Claim processing and settlement.
Injured in a motor vehicle accident
For a motor vehicle accident that results in injury:
- Log in and register for the Queensland CTP portal, OR
- Complete and return the Notice of Accident Claim Form (Non-Fatal Injury)
Please note: both versions of the Notice of Accident Claim Form (Queensland CTP portal and .PDF versions) must be printed and signed in the presence of an ‘eligible witness’. For more information, see Pre-claim checklist.
People who have lost a family member
For a motor vehicle accident where someone was fatally injured, complete the Notice of Accident Claim Form (Fatal Injury):
