Forms
Lodging a Notice of Accident Claim Form, along with supporting documents, notifies the relevant CTP insurer of the vehicle that may have been at fault and provides the information needed to assess your claim.
- All versions of the Notice of Accident Claim Form (Queensland CTP portal and PDF versions) must be printed and signed in the presence of an eligible witness. For more information, see our Pre-claim checklist.
- For fatal injury claims, please use the PDF Notice of Accident Claim Form (Fatal Injury) provided below. This claim type cannot currently be submitted through the Queensland CTP portal.
- The Pre-claim checklist also explains when you may need to complete a Report of traffic incident to police form or Additional information form.
Certificates
When lodging a Notice of Accident Claim Form, claimants may be required to complete certificates that provide important information to support their claim. These certificates help ensure transparency, verify key details, and assist insurers in assessing and processing claims accurately and fairly.
- The CTP insurance medical certificate is included in the Notice of Accident Claim Form (Non-Fatal Injury). Information on what the certificate must include and how to have it completed is in the CTP insurance medical certificate in the Pre-claim checklist.
- A Claimant certificate is included in all versions of the Notice of Accident Claim Form. For information, see our Pre-claim checklist.
Law Practice Certificate
If you have a lawyer helping with your claim, they must complete a Law Practice Certificate. If you don’t have a lawyer, you don’t need to worry about this.
