Queensland CTP portal Notice of Accident Claim Form outage: Information and updates

The Queensland CTP portal Notice of Accident Claim Form is currently offline while we fix a technical issue. You can still lodge a Queensland Compulsory Third Party (CTP) insurance claim directly with the insurer of the vehicle most at–fault for the accident.
If you need to lodge a claim, do not wait for the online form to return as timeframes apply. See Timeframes for more information. If you are concerned about a deadline, contact the insurer of the vehicle that may have caused the accident or the Motor Accident Insurance Commission (MAIC) promptly and explain your concern. MAIC can help with the lodgement process but cannot provide legal advice.
What is the technical issue?
Some information entered into this form may not have saved correctly. MAIC has paused the form while it checks the issue and helps affected people. We’re sorry for the inconvenience and any extra effort this may cause.
Other Queensland CTP portal features remain unaffected. You can still lodge provider treatment plans, claimant rehabilitation requests, and view claimant histories.
How to submit a claim during the outage
You can still submit a claim during the outage.
- Before you start: Claims require supporting documentation and information. Before you get started with your claim, review the Pre-claim checklist and ensure you have the supporting documents and information you need.
- How to submit your claim: For a motor vehicle accident that results in injury, print, complete and return the Notice of Accident Claim Form (Non-Fatal Injury). The completed form and supporting documents can be submitted to the insurer of the vehicle that may have caused the accident via post or email. See the Qld CTP insurers page for insurer contact details.
