You or your legal practitioner (if you have one) can request rehabilitation or reimbursement online through the Queensland CTP claim portal. Your rehabilitation provider can also submit a treatment plan for how they propose to help you recover from injury. Read our FAQs below or visit our help page to learn more.
If you lodged your claim form in the PDF version not the online version, please set up an account and log into the Queensland CTP claim portal. You can then request rehabilitation or reimbursement online.

- click ‘Register/login’ button and follow the instructions.
The password must have between 8 and 64 characters. The password must contain at least 3 of the following:
- a lowercase letter
- an uppercase letter
- a digit
- a symbol.

No, at this stage your password will not expire. You will only need to reset your password if you have forgotten it.
As it is an individual login, please use your individual email address. If you are using this platform in the course of your employment, please use your business individual email address.
You will need to provide either a landline or mobile phone number. If you do not have a mobile phone, you can provide a landline number and select the option to receive a call.

You can complete the request with assistance from a contributor (e.g. a legal representative, if you have one) or submit it yourself directly to the insurer.
When you ask a CTP insurer to fund rehabilitation or treatment for your injury, sometimes the insurer will ask your treating provider (e.g. physiotherapist or psychologist) to create a treatment plan. Your treating provider is responsible for completing and submitting the Online treatment plan if they are asked to do so by the CTP insurer.
No, the online request form can only be used if you have a current CTP insurance claim. If you were injured in a motor vehicle crash that occurred in Queensland, and want to claim for rehabilitation and treatment expenses, you first need to lodge a claim online or through our PDF version with the CTP insurer of the at-fault vehicle.
Using the system
To keep your personal information secure, your draft online request will remain in the system for 60 days after it was last modified. After you submit your request, it will remain on your dashboard for 21 days.
You will need your CTP claim number to submit your request. Normally you can find your claim number on correspondence from the CTP insurer. If you are unsure, you can contact the insurer and ask for your claim number.
No, please complete a separate request for each claim.
The insurer will review your request to check if it is reasonable and appropriate to approve, partially approve, or decline, or if they require more information before making a decision. They will advise you of their decision in writing within 10 days after they have received your request or within 10 days after further information is received. For any questions about your rehabilitation request, speak to your legal representative (if you have one) or contact the CTP insurer directly.

No. Once you have lodged the original rehabilitation or reimbursement request with the insurer, you will then need to communicate with the CTP insurer directly either via, email, mail or phone.
