Changes to forms

As a result of recent legislative amendments to the Oaths Act 1867 (Qld) effective from 30 April 2022, MAIC has been required to update a number of forms containing statutory declarations. All updated forms are now available on the MAIC website under Forms for CTP claims. It is important to ensure you use the correct version of the form. Should you not use the correct form, this may hold up your claim.

Viewing and completing forms

When you click a link to one of our PDF forms, it will automatically open in the PDF viewer built into your internet browser (or it will prompt you to download the file). This enables you to view and print the form. The way to print varies between different internet browsers, but generally you can press on your keyboard Ctrl + P (on PC/Windows) or Command + P (on Mac).

Completing, saving and printing interactive PDF forms

The best way to complete an interactive PDF form is to download the form then open it with Adobe Acrobat Reader. Adobe Acrobat Reader is free to download from the Adobe website if you don’t already have it on your device.

You can press the ‘tab’ key to skip between response fields in the form. Alternatively, use your mouse to click on the field you wish to edit.

All fields in the forms have a set character limit. If there is not enough space to provide your details, use the ‘Additional Information’ section at the back of the form, or attach extra pages when you lodge the form with the insurer.

Once you have completed your form, you can save it to your computer and print it. To print the form, press on your keyboard Ctrl + P (on PC/Windows) or Command + P (on Mac).

Witnessing your form

Our forms must be verified by statutory declaration and signed in the presence of an eligible witness. For further information on who can witness your form, please read our fact sheet.

Directing a substitute signatory to sign your form

If required, you can direct a person to sign a form as a substitute signatory. For further information about who can be a substitute signatory, please read our fact sheet.

If you are unable to sign your form in the physical presence of a witness

If you are unable to physically sign your form or have it witnessed in person, please contact us and we can assist you with alternate options.
